Pet Wellness Reimbursement Program Terms & Conditions

Please read these Terms and Conditions (“Terms”) carefully. They explain how the US Service Animals Pet Wellness Reimbursement Program (the “Program”) works, including how to submit a reimbursement request through the Member Portal, how much you can be reimbursed, and what is and is not eligible. By enrolling in or using the Program, you (“you” or the “Member”) agree to these Terms.

This Program is not insurance. The Program is a reimbursement benefit offered by US Service Animals to its customers. It is not an insurance policy, is not regulated as insurance, and is not sold or administered by a licensed insurance company or agent. It does not provide coverage for illness, injury, accidents, or emergency veterinary care. If you want insurance-style coverage for unexpected veterinary costs, consider a licensed pet insurance policy in addition to this Program.

1. Program Overview

The Program reimburses enrolled Members for eligible, routine wellness expenses incurred for up to three Eligible Pets, up to fixed annual maximums, after the Member pays a licensed veterinary provider directly and submits documentation through the Member Portal for our team’s review.

The Program is offered as a benefit of your relationship with US Service Animals and is subject to these Terms, which US Service Animals may update from time to time as described in Section 14 (Changes to the Program). These Terms are part of, incorporated into, and should be read together with the US Service Animals Terms of Service and Privacy Policy (together, the “General Terms”).

2. Definitions

  • “Eligible Pet” means a dog or cat owned by the Member, enrolled in the Program, and identified in the Member’s account on the Member Portal.
  • “Eligible Expense” means an expense described in Sections 5 and 6 that meets the requirements of these Terms.
  • “Member Portal” means US Service Animals’ online customer portal through which Members submit reimbursement requests and supporting documentation.
  • “Program Year” means, for each Eligible Pet, the rolling 12-month period beginning on the date US Service Animals first charges the VIP Elite fee for that Eligible Pet, and each subsequent 12-month period, as described in Section 10.
  • “Reimbursement Request” means a request for reimbursement submitted through the Member Portal together with supporting documentation.
  • “We,” “us,” “our,” and “US Service Animals” refer to US Service Animals.

3. Eligibility

To participate in the Program, you must:

  1. Have an active VIP Elite membership from US Service Animals in good standing;
  2. Enroll each dog or cat for which you wish to receive reimbursement as an Eligible Pet through the Member Portal; and
  3. Be the pet’s owner or authorized caretaker and the person who paid for the veterinary services for which you are requesting reimbursement.

US Service Animals may, in its discretion, set additional eligibility criteria (for example, a minimum or maximum pet age, a limit on the number of Eligible Pets per account, or geographic restrictions) and will post any such criteria on the Member Portal or in your account materials.

4. Benefit Summary

Each Eligible Pet may receive reimbursement up to the following annual maximums. The two categories are tracked separately and are not interchangeable. Unused amounts in one category cannot be applied to the other.

Example: If an Eligible Pet’s owner spends $90 on a wellness exam and flea/tick prevention and $60 on vaccinations in the same Program Year, US Service Animals would reimburse $70 (the wellness maximum) plus $45 (the vaccination maximum) — $115 total — not $150.

5. Eligible Expenses: General Wellness

Subject to the annual maximum in Section 4, General Wellness reimbursements cover the following:

  • Annual or wellness veterinary examinations
  • Routine bloodwork, fecal, heartworm, and other standard wellness screening tests
  • Flea, tick, and heartworm preventives
  • Routine dental cleanings
  • Spay or neuter procedures
  • Microchipping

6. Eligible Expenses: Vaccinations

Subject to the annual maximum in Section 4, Vaccination reimbursements cover core and non-core vaccines administered by a licensed veterinary provider. This includes rabies, DHPP/DAPP (dogs), FVRCP (cats), Bordetella, leptospirosis, and feline leukemia (FeLV).

7. Expenses Not Eligible for Reimbursement

The following are not reimbursable under the Program, regardless of category:

  • Diagnosis or treatment of illness, injury, or accidents (including emergency and urgent care, hospitalization, surgery not listed in Section 5, and prescription medications for treating an existing condition)
  • Expenses incurred before the pet’s enrollment date or after cancellation of the Program
  • Boarding, grooming not tied to a medical need, training, pet food, treats, and non-prescription supplements
  • Expenses for a pet that is not an Eligible Pet on your account at the time of service
  • Expenses submitted without the documentation required under Section 8, or submitted after the deadline in Section 8
  • Expenses paid or reimbursed by another party, including a pet insurance policy, another wellness or discount plan, or a veterinary clinic promotion (see Section 11 for coordination with other benefits)
  • Sales tax, convenience fees, or finance charges billed separately from the veterinary service itself

US Service Animals determines, in its reasonable discretion, whether a submitted expense qualifies as an Eligible Expense.

8. How to Submit a Reimbursement Request

To request reimbursement:

  1. Pay your veterinary provider directly for the service.
  2. Log in to the Member Portal and upload a copy of your itemized receipt or invoice.
  3. Your receipt or invoice must clearly show: the pet’s name, the date of service, an itemized description of each service or product, the amount charged and amount you paid, and the name of the veterinary provider or clinic.
  4. Submit the Reimbursement Request within 90 days of the date of service. Requests submitted after this window may be denied.
  5. Respond promptly if our team requests additional information or a clearer copy of your documentation. Requests may be closed if we do not receive a response within 30 days of our request.

You are responsible for the accuracy and completeness of everything you submit. Do not alter, obscure, or combine receipts from multiple pets or providers into a single upload.

9. Review and Reimbursement Timeline

Our team generally reviews complete Reimbursement Requests within 7 business days of submission. If approved, reimbursement is issued within 5 business days of approval, up to the remaining balance of the applicable annual maximum. US Service Animals issues reimbursements by ACH transfer to the account you designate, or by another method we may offer, such as a check or a credit to your US Service Animals account. You are responsible for providing accurate payment information, and US Service Animals is not responsible for delays or misdirected payments resulting from inaccurate information you provide.

10. Program Year and Cap Reset

Each Eligible Pet has its own Program Year, which begins on the date US Service Animals first charges the VIP Elite fee for that pet and renews automatically every 12 months for as long as the pet remains enrolled. Annual maximums reset to the full amount at the start of each new Program Year. Unused amounts do not carry over or accumulate into a future Program Year, and reimbursements cannot be advanced against a future Program Year.

11. Multiple Pets and Coordination with Other Benefits

Each additional Eligible Pet increases your VIP Elite fee, as disclosed at enrollment and in your account. If you enroll more than one Eligible Pet, each pet has its own separate annual maximums under Section 4. Those maximums do not combine or transfer between pets. If an expense is also eligible for payment under a pet insurance policy or another wellness or discount program, US Service Animals will only reimburse the portion of the expense not covered or reimbursed by that other source, and you must disclose any other applicable coverage when submitting your Reimbursement Request.

12. Appeals

If your Reimbursement Request is denied or approved for less than you expected, you may request a review by submitting a written appeal to [email protected] within 30 days of the decision, along with any additional documentation supporting your request. We will notify you of the outcome of the appeal within 15 business days of receiving it. This decision is final under the Program’s internal appeal process.

13. Fraud, Misrepresentation, and Program Integrity

US Service Animals may take any one or more of the following actions if we reasonably believe a submission is fraudulent, altered, duplicated, or materially misrepresents the services rendered, the amount paid, or the pet or provider involved:

  • deny a Reimbursement Request;
  • require additional verification;
  • suspend or terminate your participation in the Program; or
  • pursue any available legal remedy.

14. Changes to the Program

US Service Animals may modify these Terms, the annual maximums, the list of Eligible Expenses, or any other feature of the Program, or discontinue the Program altogether, at any time. We will post material changes on the Member Portal or notify you through your account at least 30 days before they take effect, except where a change is required sooner by law or is needed to prevent fraud or abuse. Continued participation in the Program after a change takes effect constitutes acceptance of the updated Terms.

15. Cancellation and Termination

You may stop participating in the Program at any time by removing a pet from enrollment or canceling your VIP Elite account through the Member Portal. US Service Animals may also suspend or terminate your participation, including for the reasons described in Section 13, for non-payment of any associated account fees, or if we discontinue the Program under Section 14. Reimbursement Requests for services performed before the effective date of cancellation remain subject to the submission window in Section 8; expenses incurred after cancellation are not eligible.

16. This Is Not Insurance

As stated above, the Program is a reimbursement benefit, not an insurance policy. US Service Animals is not a licensed insurer or insurance producer with respect to the Program, does not guarantee reimbursement of any particular expense, and the Program does not create a right to coverage beyond the fixed annual maximums described in Section 4. Nothing in these Terms should be read as a promise to pay for veterinary treatment of illness, injury, or emergencies.

17. Privacy

Information you submit through the Member Portal, including receipts and other documentation, is handled in accordance with our Privacy Policy. By submitting a Reimbursement Request, you consent to our use of that information to administer the Program, including reviewing, verifying, and processing your request and detecting fraud.

18. Limitation of Liability

To the fullest extent permitted by law, US Service Animals’ total liability arising out of or relating to the Program will not exceed the total amount of reimbursement actually owed to you under Section 4 for the Program Year in question. US Service Animals is not liable for indirect, incidental, consequential, or punitive damages arising from the Program, including delays in review or payment, technical issues with the Member Portal, or a veterinary provider’s records or billing practices. Nothing in these Terms limits any liability that cannot be limited under applicable law (for example, liability for fraud, gross negligence, or willful misconduct). Some jurisdictions do not allow certain of these limitations, so parts of this Section may not apply to you.

19. Indemnification

To the fullest extent permitted by applicable law, you agree to defend, indemnify, and hold US Service Animals harmless from any claim, loss, or expense (including reasonable attorneys’ fees) arising from a Reimbursement Request you submit that is fraudulent, materially inaccurate, or violates these Terms.

20. Governing Law

These Terms are governed by the laws of the State of Louisiana, without regard to its conflict-of-laws rules.

21. General Provisions

  • Entire Agreement; Relationship to General Terms. These Terms supplement and form part of the General Terms. Together, they are the entire agreement between you and US Service Animals regarding the Program. If these Terms conflict with the General Terms on a matter specific to the Program, these Terms control. The General Terms govern all other matters, including dispute resolution, arbitration, class-action waiver, and venue.
  • Severability. If any provision of these Terms is found unenforceable, the remaining provisions remain in full force and effect.
  • No Waiver. US Service Animals’ failure to enforce any provision of these Terms is not a waiver of that provision.
  • Assignment. You may not assign these Terms or your Program enrollment. US Service Animals may assign these Terms in connection with a merger, acquisition, or sale of assets.
